Instead of asking others to make one for you, why don't you make one yourself? It's actually very simple and lucky for you, I've recently written a tutorial to teach others how to make one themselves. It's very simple to read and the instructions are easy to follow. You should be finished with your DIV overlay in about 20 minutes (at the MOST).
